We ask that you either come 15 minutes early to your appointment to fill your forms out in person or you may print them out at home and have them filled out and ready for your appointment time.
If it's your first time getting a facial, wax, massage, body treatment, or facial add on we have all clients fill out a consent form. We may also ask that you fill one out if you have not had one of these services in a while. This form helps us understand you better as well as help you understand the service.
